Curtain or Shadow in Your Vision? Know the Warning Signs

A curtain, shadow, flashes or a sudden increase in floaters can be a warning sign of a retinal tear or detachment. Learn when to seek urgent care.

Warning signs
!
Prompt examination can protect vision.
A sudden curtain, shadow, dark patch or veil across your vision can be frightening. Sometimes visual disturbances are harmless, but a new curtain-like shadow can also signal a retinal tear or retinal detachment — conditions that need prompt examination.

What does a curtain or shadow feel like?

People describe this warning sign in different ways: a grey curtain coming down from one side, a dark shadow spreading across peripheral vision, or a fixed dark area. A retinal detachment may begin in the peripheral retina, so the first symptom can be a loss of side vision.

A curtain-like symptom is particularly concerning when it appears suddenly or is accompanied by new flashes and many new floaters. Symptoms alone cannot safely distinguish a harmless change from an emergency.

Why is the retina so important?

The retina acts like the image-sensing layer of a camera. Specialised retinal cells convert light into signals that travel through the optic nerve to the brain. When the retina is torn, pulled, swollen, bleeding or detached, the way you see can change suddenly.

What is retinal detachment?

Retinal detachment happens when the retina separates from the layers that support it. Without prompt treatment, the detached area may lose function and central vision can become affected. A retinal tear can sometimes be treated before a major detachment develops.

How is it diagnosed and treated?

Diagnosis usually includes a detailed history, vision testing, pupil examination and a dilated retinal examination. OCT or ultrasound may be used when the view is limited or more information is needed.

Treatment depends on the type, location, size, duration and severity of the retinal problem. A tear may be sealed with laser or cryotherapy. Retinal detachment may require a procedure such as pneumatic retinopexy, scleral buckle or vitrectomy.

When should you seek urgent care?

  • A new curtain, shadow or dark patch in your vision
  • A sudden increase in floaters or specks
  • Flashes of light, especially in the peripheral vision
  • Missing areas of side vision or a sudden change in one eye
  • Blurred or distorted vision after eye trauma
If these symptoms appear, do not wait to see if they settle. Prompt retinal examination can protect vision when a tear is found early.
